A persistent large infected vegetation can be seen by intracardiac echocardiography after an infected transvenous right ventricular pacing lead was extracted. The vegetation was most likely still adherent to an adjacent PICC line. A channel can be seen in the center of the vegetation through which the lead had previously passed, giving the appearance of a kabob after being pulled off the skewer. (courtesy of Dr. Al Lin.)
